Your First Visit

To better prepare you for your first visit we suggest the following:

  • Arrive 10 minutes before your scheduled appointment to allow time to complete any necessary forms.
  • Bring or wear comfortable clothes, which allow your therapist to assess the area of concern. We do have hospital gowns available.
  • Bring your insurance information for billing purposes.
  • If you have a referral bring it along. Insurance varies and some require a referral for us to direct bill, or for you to submit for repayment. We suggest you confirm with your insurance company before attending.
